Responsibilities:
Develop social histories and care plans for residents.
Participate in interdisciplinary care plan conferences.
Provide individual and group support for residents and families.
Coordinate discharge planning and educational programs.
Requirements:
Bachelor’s Degree in Social Work (BSW) or equivalent.
Effective communication skills.
Long-term care experience.
