We are seeking a Licensed Speech-Language Pathologist (SLP) to join our innovative care coordination program, focusing on improving the lives of seniors in various living settings. This role allows you to provide speech-language pathology services while engaging in care coordination tasks, offering flexible scheduling and career growth opportunities.

Responsibilities include:

• Delivering speech-language pathology and wellness services to older adults.
• Conducting assessments and therapeutic interventions as part of a multidisciplinary team.
• Supporting care coordination through wellness visits and communication with providers.
• Maintaining professional standards and contributing to program development.

Qualifications:

• Completion of an accredited SLP program (New Grads Welcome).
• Current state licensure and ASHA certification.
• Strong communication skills and a results-oriented mindset.
• Tech-savvy with problem-solving abilities.
