Grand Oaks Nursing & Rehabilitation Center is hiring a Clinical Care Coordinator to provide clinical oversight and assess residents’ physical and mental functions.
**Wage Information:** $12,000 sign-on bonus, $44.45-$50.45/hour based on experience, plus MI pass-through pay of $0.85/hour until 09.30.25. **Responsibilities:** Conduct assessments, document data, make referrals, supervise resident care, and manage infection prevention programs. **Requirements:** Must be a Registered Nurse (RN) with strong customer service and interpersonal skills. **Benefits:** Include 401(k), health insurance, paid time off, and more.
