Clinic Director – Licensed Therapist OT/PT – Care Coordination

Job Description

We are seeking a Licensed Clinic Director for a Care Coordination Program focused on enhancing the lives of seniors in various living environments. This role allows licensed therapists to practice at their highest level while engaging in care coordination tasks. Responsibilities include overseeing patient care, providing therapy services, and contributing to program development and quality improvement. Candidates should have a degree in OT, PT, COTA, PTA, or SLP, relevant state licensure, and strong communication skills.
