Location: Waukesha area
Full-Time: Monday – Friday, no nights, weekends, or holidays

Position Overview: Seeking an experienced Advanced Practice Provider (NP/PA) in internal medicine to provide onsite medical care in skilled nursing facilities, focusing on acute and chronic care for adult and geriatric patients.

Key Responsibilities:

  • Deliver primary care and develop personalized treatment plans.

  • Establish diagnoses, prescribe medications, and perform procedures.

  • Document patient assessments and coordinate with specialty providers.

  • Communicate effectively with patients, families, and the clinical team.

  • Foster relationships to enhance care quality.

    • Qualifications:

  • Valid NP/PA license in Wisconsin, DEA, and AHA BLS certification.

  • Minimum 1 year of experience preferred, especially in skilled nursing.

  • Strong diagnostic and interpersonal skills.
