Job Description

> REF:#f8165f7440f6ee4e *Therapist & Counselor Qualifications:*
* Master’s degree and become licensed as LSW or LPC within one year of employment or SWT/CT licensure
* Must have NPI number and an Ohio Medicaid number
* Must have excellent skills in diagnostic assessment and psychotherapy
* Must be able to pass a drug/alcohol screening

*Preferred Therapist & Counselors:*
* LPCC or LISW preferred
* CAQH account (if independently licensed)

*Job Duties of Therapist & Counselors:*
* Diagnostic assessments
* Counseling/psychotherapy for individuals and/or groups
* Referral/linkage with other services
* Documentation for provided services
