Job Description

Type of Facility: Home Health Agency
Length of Assignment: 13 weeks (with potential for extension)
Start Date: Immediately
Work Schedule: Monday-Friday, 9am-5pm or 4 days a week
Roles and Responsibilities:
– Conduct initial evaluations, starter care, and discharge visits for home health patients
– Manage a caseload of 6-7 patients per day
– Provide physical therapy services to patients aged 60 and up, primarily Medicare beneficiaries
– Collaborate with PTAs who manage ongoing treatment plans
– Maintain accurate and detailed patient records
– Complete documentation using personal smartphone or laptop
Requirements:
– Active California PT license
– At least 1 year of PT experience
– BLS + CPR certification
– Valid driver’s license, car insurance, and vehicle registration
– Comfortable using technology for documentation and communication
