As a full-time Home Health Physical Therapist, you’ll play an essential role in improving the quality of life for our patients, providing care that blends professionalism with a personal touch. Whether you’re assessing patients’ needs, helping them achieve functional goals, or providing education to their families, your work will leave a lasting impact.
*Responsibilities:*
* Provide comprehensive patient care by assessing their condition at each visit and documenting your findings with precision and care.
* Evaluate a range of health factors, including vital signs and daily living activities, ensuring each patient receives tailored, top-quality care.
* Use your physical therapy expertise to develop rehabilitation and therapy plans that enhance patients’ functional status and overall well-being.
* Recommend appropriate equipment or tools to improve patients’ quality of life and aid in their recovery process.
* Actively listen to patients’ concerns and requests, addressing their needs with compassion and understanding.
* Work collaboratively with a multidisciplinary team, including Registered Nurses, Occupational Therapists, Physicians, and other healthcare professionals, to develop personalized care plans for each patient.
* Educate patients and their families on effective home health strategies to empower them in their journey toward recovery.
*Requirements:*
* A compassionate, patient-centered approach in all interactions.
* Excitement for exploring the dynamic neighborhoods of the Mid Valley area while visiting multiple patients each day.
* A valid Physical Therapy license from the Physical Therapy Board of California.
* Prior experience in home health or hospice care is a plus, but above all, we value passion, dedication, and a commitment to patient care.
* Excellent communication skills and time management abilities, balancing efficiency with a personalized touch.
* A valid driver’s license and reliable transportation.
* Current CPR certification and up-to-date TB clearance.
*Experience:*
* A minimum of 1 year of experience in Physical Therapy is required.
*License/Certification:*
* A valid Physical Therapy license is essential.
