RESPONSIBILITIES
· Registered Nurse (RN) position is responsible for assessing and evaluating patients
· Writing and initiating plan of care
· Regularly re-evaluating plan of care
· Revising the plan of care as necessary
· Administer medication and treatment as prescribed by the physician
· Document accurate and relevant clinical notes regarding patient’s condition
· Observes patient’s change in condition and behavior, discuss changes with the patient and/or family/caregiver, and reporting when needed
· Assists patients with activities of daily living and facilitates the patient’s efforts towards self-sufficiency and independence
REQUIREMENTS
· Active and good-standing RN license in the state of Pennsylvania; BSN preferred
· At least 1 year experience within Home Health, Clinical, Surgical, or Critical Care environment
· Valid driver’s license with reliable, insured transportation
