Home Care Case Manager – Registered Nurse (RN)

Job Description

> REF:#b4352624057f9d02 Tallavera is currently seeking a Registered Nurse (RN) Home Care Case Manager in Rochester, NY. Preferred candidates for the Registered Nurse (RN) Home Care Case Manager position will be a New York State Licensed Registered Nurse, with 1 year experience as a clinical nurse in a hospital or other direct patient care clinical setting.

*Responsibilities of the Registered Nurse (RN) Home Care Case Manager:*

* Assesses and charts observations of the patient’s condition at each visit
* Revises plan of treatments and discharge plan in conjunction with registered nurse case manager, patient, family, physician, care delivery team, and third-party payors including obtaining medical orders and approval of medical services.
* Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.

*Requirements of the Registered Nurse (RN) Home Care Case Manager:*

* NYS driver’s license
* NYS Licensed Registered Nurse
* OASAS certification preferred
