*Puget Sound Home Health and Hospice *is seeking an experienced and caring full-time *Hospice Registered Nurse Case Manager (RN)* in *Pierce County, Washington! *

*Essential Duties*
* Completes comprehensive in-home patient assessment
* Oversees comfort care and pain management
* Develops plan of care under primary care providers orders
* Provides direct patient care in accordance with the plan of care and scope of practice
* Provides accurate & timely documentation consistent with plan of care
* Participates in QAPI program to assure highest level of patient care
* Participates in interdisciplinary coordination of home health and hospice services

*Requirements and Qualifications*
* Registered Nurse (RN) licensure
* *Prior experience working with hospice patients is required*
* Ability to work independently and demonstrate critical thinking
* Current BLS certification from AHA
* Be a licensed driver with an insured automobile in good working order
