Advanced Health Care, is currently hiring full time and per-diem therapists responsible for completing therapeutic evaluation assessments and corresponding treatments for patients in their place of residence. PTs will have direct contact with the referring SNF and in-house therapists to become acquainted with the primarily orthopedic patients prior to following them in the home. Will work with the Nurse Case Manager to ensure interdisciplinary care coordination. *Active PT license required.* *Home Health experience preferred* _(However we are looking for great people that want to work hard, and will give people the opportunities and skills to succeed in our company)._
