LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community-based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric physicians and nurse practitioners, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants’ needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in-home program according to an individual Participant’s needs.

Description
Responsible for working directly with the medical services staff to assist with patient exam and care.
• Assists with the flow of physician services in each center as directed by the physician, Nurse Manager or Director of Clinical Services including: preparing participants for the physician’s examinations, obtaining monthly (and as ordered by the physician) vital signs, weights, and record on Vital Sign and Weight Flow Sheet.
• Supports the Medical Services Staff by obtaining specimens, emptying specimen containers such as foley, ostomy bag; provide perineal care, including but not limited to foley catheter and leg bag care, performing glucometer checks; calibrating the meters with test solution monthly, application of assistive devices.
• Although the primary focus of this position is to support the medical staff, when time permits, this position will also assist the nurse with dressing changes, wound care, ostomy care, maintain exam room and par stock supplies on a daily basis, answer telephone in the Medical Services Area when the staff are at meetings or occupied with participant care, record messages on the log provided, and demonstrate basic computer skills; for example, email, ordering supplies, forward/attaching documents.
• Promote optimal health and maximize independence and self-care for the frail elderly, in accordance with LIFE Pittsburgh philosophy.
• May need to provide basic Personal Care services to participants; including but not limited to, bathing, grooming, toileting, feeding.
* Assists in keeping the Center clean and in an orderly condition.
* Reports observations of participant status to the nurse and the physician
* Maintains records of work performed as required by LIFE Pittsburgh.
* Maintain participant privacy and dignity as all times.
May perform additional essential and/or non-essential job functions as assigned by supervisor at any given time with or without notice.
Requirements:
Valid CNA license (preferred) or comparable certificate/experience. Valid Driver’s License, access to a vehicle and valid automobile insurance required. At least One year of experience with the frail elderly.
