Heritage Rehab is seeking a skilled Supervising Physical Therapist to join our *In- House Therapy Team*! Looking for part-time; at least 3 days/ week (15-25 hours/ week or more) of PT coverage. This opportunity also allows for a *full-time split SALARIED* position w/ our building in State College (Hearthside)-inquire for more details.

_*Our skilled nursing facility located in Orbisonia is not far from I-76. *_

**Duties**

– Conduct assessments and develop personalized treatment plans for patients

– Provide hands-on therapy and exercises to improve patients’ mobility and relieve pain

– Educate patients and their families on exercises and self-care techniques

– Collaborate with other healthcare professionals to ensure comprehensive patient care

– Maintain detailed patient records and progress reports

**Experience:**

– Experience in geriatrics, acute care, or inpatient settings is preferred

– Familiarity with medical terminology

– Ability to work effectively in a fast-paced environment
