– Provide physical therapy services to patients, including evaluation, treatment planning, and implementation of treatment plans.
– Develop and implement individualized care plans based on patient needs and goals.
– Monitor and document patient progress, adjusting treatment plans as necessary.
– Collaborate with other healthcare professionals to ensure comprehensive patient care.
– Educate patients and their families on exercises and techniques for at-home care.
– Stay up-to-date with advancements in physical therapy techniques and research.

Requirements:
– Valid license to practice as a Physical Therapist (PT).
– Experience in Home Health is preferred.
– Knowledge of medical terminology, anatomy, and physiology.
– Strong interpersonal and communication skills to effectively interact with patients and healthcare team members.
– Ability to assess patient needs and develop appropriate treatment plans.
– Compassionate and patient-centered approach to care.

Job Type: Full-time

Pay: $70.00 – $80.00 per hour
