SUMMARY
Ensures the health and safety of individuals by assessing individuals for specific nursing needs.
Provides prescribed medical treatment and personal care to individuals in their home setting, performing the following duties. Complies with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Coordinates and supervise the activities of all licensed clinical team members for the individuals served under the supervision of a physician or healthcare provider for the diagnosis, treatment and prevention of disease. All clinical team members include Registered Nurses and Licensed Practical Nurses.
Establishes clinical team member work schedules on a weekly basis. Responsible for coordination of coverage including temporary and substitute scheduling.
Monitors daily performance of clinical team members to ensure duties are completed accurately, efficiently and timely.
Reviews, corrects, and approves timesheets, reviews and responds to time off requests and coordinates for clinical team members.
Conducts all clinical team interviews. Coordinates all clinical team orientation, education, and training on the program and individual-specific clinical protocols, policies and procedures.
Identifies and recommends solutions to clinical team members issues or problems. Participates in all required meetings necessary for the support of the persons who live in the home. Will be actively involved in leading communication with the families of the persons served for all medical supports.
Ensures high quality clinical care for the individuals served. Provides frequent skilled nursing assessment, education, and treatment to individuals served as ordered by the healthcare provider.
Responsible for ensuring all medical concerns or complaints are reported to the appropriate healthcare provider within a reasonable time frame.
Responsible for arranging follow up with healthcare providers when necessary. Attends or arranges for nursing support for all medical appointments for each of the individuals.
Maintains adequate clinical supplies and medication inventory. Ensure clinical equipment remains in operating condition.
Ensures program compliance with DPH MAP Policy.
Ensures clinical policies and procedures comply with current Lifeworks, state and federal regulations.
Provides educational training, counseling, and physical assistance when necessary to achieve ISP goals, and provide regular in-service training.
Responsible for establishing relationships with nursing staffing agencies when necessary to provide substitute/emergency coverage for the program.
Attends meetings, in-services, ISP’s, significant medical appointments, and case conferences as necessary
SUPERVISORY RESPONSIBILITIES
This person will supervise all clinical team members as defined in this job description. This includes Registered nurses and Licensed Practical Nurses, as well as all temporary substitute staffing in these positions. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning and scheduling of overall program nursing coverage, directing clinical duties; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

EDUCATION and/or EXPERIENCE
Bachelor’s Degree in Nursing and Licensed Registered Nurse with at least five years of nursing management experience. An Associate’s Degree and Registered Nurse license will be accepted with 10 years of demonstrated experience as listed here.
LANGUAGE SKILLS
Ability to read, analyzes, and interprets general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions form groups of managers, families and/or guardians of Individuals served.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write and proofread routine reports and correspondence. Ability to speak effectively before groups of individuals or employees of an organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY
Ability to apply common-sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
VEHICLE/TRAVEL
Requires a legally insured and inspected automobile for use to be presented at work and available for use during working hours. If the program has an agency vehicle, staff are expected to drive it as needed.
CERTIFICATES, LICENSES, REGISTRATIONS
Current Massachusetts RN license, valid and current driver’s license from the state of residency, MAP.
