Job Description

Type of Facility: Home Health Agency
Length of Assignment: 13 Weeks (open to 26 weeks)
Start Date: Early January
Work Schedule: 8-10 hour shifts for 4-5 days per week
Roles and Responsibilities:
– Provide in-home physical therapy services to homebound geriatric patients
– Conduct assessments, evaluations, and treatments
– Develop and implement initial and ongoing treatment plans
– Create discharge plans and supervise assistants/aides
– Manage a caseload of 30-32 patients per week
– Schedule and coordinate patient visits independently
– Communicate treatment plans with patients’ families and other healthcare professionals
– Adapt to different work environments and be tech-savvy
– Handle occasional cancellations and follow-up as needed
Requirements:
– Florida PT license
– CPR certification
– Valid driver’s license, car registration, and insurance
– At least 1 year of home health experience
– Own smartphone, tablet, or laptop for documentation
– Adaptability and strong communication skills
– Lymphedema experience and bilingual skills are a plus
