Position Details
Shift: Flexible, 30 hours per week
Key Responsibilities
Develop preliminary and comprehensive dietary assessments tailored to each resident’s needs.
Create individualized dietary care plans that address resident-specific problems and set achievable goals.
Collaborate with residents and their families to review and refine dietary care plans.
Guide dietary personnel in implementing care plans and ensure they understand and adhere to the specific requirements of each resident.
Identify and document any special equipment or utensils residents may require for meal consumption.
Analyze patient charts and provide informed recommendations for medical nutrition therapy.
Qualifications
Licensure: Licensed Registered Dietitian in the state of Pennsylvania.
Experience:
Minimum of two years as a professional dietitian in long-term care, including administrative responsibilities, or
At least one year as a consulting dietitian for one or more long-term care facilities.
