Join our _*in house therapy team*_ at Heritage Rehab as a Physical Therapist dedicated to providing exceptional care. We are looking for a passionate individual with expertise and a commitment to promoting physical health.



Our skilled nursing facility located in Madison, Ohio provides an inviting, home-like environment conveniently located near I-90. This 120-bed facility has private rehabilitation suites with easy access to the therapy gym. The therapy staff and interdisciplinary team strive to create a comprehensive care plan that is unique to each short-term, outpatient, or long-term patient.



**Responsibilities:**

– Conduct comprehensive assessments to develop personalized treatment plans for patients.

– Administer therapeutic activities and exercises to enhance patients’ functional abilities.

– Educate patients and their families on physical therapy’s plan of care

– Collaborate with interdisciplinary teams

– Maintain accurate documentation of patient progress and treatment plans.

**Qualifications:**

– State licensure as a Physical Therapist.

– Excellent communication skills and ability to work effectively in a fast-paced environment.

Job Type: Part-time

Pay: From $55.00 per hour

Expected hours: 10 – 29 per week
