The VNA is seeking a Clinical Educator to work in our Indian River County Home Health division. This position will be responsible for the coordination and management of clinical education for the home health program to ensure quality and patient satisfaction for all patients and their families. This position is responsible for the Education of the Home Health associates including the orientation process and ongoing educational needs . Coordinates Education activities with the Patient Care Managers. Works in collaboration with the Patient Care Managers to provide education to staff members as needed.
Responsibilities:
Develops, implements and evaluates clinical education programs.
Identifies clinical education needs through needs assessments, physician consultation and associate’s requests for educational offerings that are specific to patient care.
Facilitates learning by collaborating with attending physicians, department heads and associates.
Facilitates learning by understanding adult learning principles and incorporating those principles into learning modules.
Maximizes care skills by increasing the associate’s ability to render care on their own without assistance.
Provides clinical education programming that individualizes education provide to the associate that will maximize the associate’s ability to improve the health and provide for optimal lifestyle for the patient.
Improves the quality of care by redesigning process and implementing change.
Organizes the clinical education programs to be effective and efficient.
Participles in the Home health QAPI
Stays current on Medicare COPS ., state and federal regulations and the Joint Commission standards
Tailors educational preparation for clinical staff in compliance with Federal and state regulations and The Joint Commission
Directs the activities of the home health clinical education program in collaboration with PCMs and VP of Clinical Services
Packages all clinical educational programs and utilizes all resources available to coordinate clinical education programming.
Will be the sole educator for the Home Health IRC program.
Qualifications:
Current RN license in the State of Florida
Bachelor of Science in Nursing (BSN) Preferred or equivalent combination of education and experience
Minimum of 3 years’ experience as an RN; with at least two years in Home Care
Previous supervisory experience preferred
Knowledge of federal and state related to home care
OASIS Certified preferred
Knowledgeable in electronic record systems, specifically in home care-specific electronic health record systems.
