Duties: – Administer medications and treatments as prescribed by physicians – Monitor and record patient vital signs, including temperature, pulse, and blood pressure – Assist with patient assessments and provide input for care planning – Collaborate with healthcare professionals to develop and implement patient care plans – Perform basic nursing procedures such as wound care, catheterization, and tracheostomy care – Provide health coaching and education to patients and their families – Assist with case management and coordination of healthcare services – Utilize electronic medical records (EMR) systems to document patient information accurately Skills: – Proficient in case management principles and practices – Knowledge of durable medical equipment (DME) usage and maintenance – Experience in emergency room (ER) settings is a plus – Familiarity with assisted living facilities and long-term care settings – Competent in IV infusion therapy techniques – Ability to provide person-centered care with empathy and compassion – Skilled in taking accurate vital signs measurements – Knowledge of ventilator management for patients requiring respiratory support – Strong health coaching abilities to support patients in achieving their wellness goals – Familiarity with ICD-10 coding system for accurate documentation If you are a dedicated LPN looking for an opportunity to make a difference in the lives of others, we encourage you to apply.
