Faith Home Health & Hospice is currently seeking a compassionate, well-trained hospice registered nurse dedicated to the highest standards of care and professionalism to work for our hospice agency in Wichita, KS. As a hospice Registered Nurse or RN Case Manager for Faith, you will provide one-on-one skilled nursing care to patients using a comprehensive, patient-specific care plan which establishes goals based on nursing diagnosis and incorporates nursing services, treatments and preventative procedures for our terminally ill patients in hospice.

We are currently looking for a Full-Time Registered Nurse (RN) to provide care to our patients in the Wichita, KS area. Additional information regarding territories to be provided during the interview process.

RN RESPONSIBILITIES:

As a Hospice Registered Nurse (RN), you will be responsible for but not limited to:
* Case managing and developing a plan of care for each assigned patient.
* Assessing patients’ conditions, initiating the plan of care, re-evaluating and updating patients conditions.
* Promoting patient/family/caregiver education taking into account the patient’s needs and the standards of care.
* Responsible for wound care, disease management, and medication reviews in the home setting.
* Functioning as a team member by collaborating with the patient, physician and interdisciplinary team.

RN QUALIFICATIONS:
* *Have a unencumbered current, valid Kansas State Board of Nursing RN license (compact).*
* Two years of experience as a registered nurse (RN) in a clinical setting preferred.
* *At least one year of prior hospice experience required*.
* Possess a valid driver’s license and reliable transportation.
* Have a current CPR certification.
* Be able to work Monday to Friday plus on call rotation.

Job Type: Full-time

Pay: From $80,000.00 per year
