Job Location: Arizona

Type of Facility: Skilled Nursing Facility

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Monday-Friday, 8:00 AM – 5:00 PM

Roles and Responsibilities:

– Provide physical therapy services to 8-10 patients daily (occasionally up to 11-12)

– Maintain a minimum productivity of 80%

– Conduct therapy sessions in 30-45 minute time slots

– Participate in group therapy sessions when required

– Treat post-acute patients recovering from various surgical procedures

– Work with both inpatient and outpatient cases, depending on patient needs

– Utilize NetHealth EMR system for documentation

– Adapt to patient stays ranging from 7-28 days based on surgical procedures

– Possibility of occasional Saturday overtime to manage weekly overload

Requirements:

– Active Arizona PT license (or compact state license, 12-16 weeks processing time)

– BLS certification

– Minimum 1 year of experience preferred
