Job Description

• Assess, coordinate, plan, direct, implement, and manage patient needs throughout their care episode, focusing on discharge progress and transitions within the acute stay.

• Ensure the delivery of competent, compassionate patient care by the caregiving team across inpatient, ambulatory, and community settings.

• Responsible for overseeing patient care provided to customers, including referring facilities, hospital staff, and patients.

• Work a night shift in the secure unit with a salary range of $68,640.00 – $70,699.20 (hourly $33.00 – $33.99).

• Maintain confidentiality and ensure access to sensitive information complies with HIPAA and applicable regulations, using the minimum necessary data for duties.
