Job Description
A Clinic Physical Therapist position in Greenfield, Wisconsin, offering ongoing employment for 36 hours weekly with day shifts.
Responsibilities include interpreting physician referrals, conducting evaluations, developing and implementing individualized treatment plans, monitoring outcomes, and adjusting care as needed.
Provides patient discharge planning, education, and community referrals, while maintaining thorough documentation and adhering to billing policies.
Ensures safety, cleanliness, and proper equipment use, and participates in quality improvement activities.
May supervise assistants, rehab aides, or students, delegating tasks and providing guidance.
Requires a Doctorate in Physical Therapy, licensure in the state, BLS certification, and excellent communication and organizational skills.
Must be able to lift and move patients and equipment safely, with physical ability to perform manual techniques and operate therapy devices.
