Job Description

The Case Manager is an active member of the healthcare team responsible for facilitating internal and external healthcare services.

  • Integrates Utilization Management, Case Management, and Discharge Planning roles.

  • Coordinates patient care and develops discharge plans through collaboration with healthcare providers and resources.

  • Promotes a multidisciplinary care model to achieve optimal clinical and financial outcomes while ensuring regulatory compliance.

    • Qualifications:

  • Minimum of an Associate’s degree required; Bachelor’s preferred.

  • Skills include effective communication, critical thinking, independence, and teamwork for discharge planning.

  • Must hold an active RN license in North Carolina or a compact state; BLS optional.
