Job Description
The RN Case Manager leads complex discharge planning and collaborates with healthcare team members to assess, plan, implement, and evaluate safe discharge strategies.
They work with social workers on psychosocially complex cases and facilitate care coordination.
Accountable for managing care in accordance with organizational policies, mission, and values.
Serves as a resource for discharge planning and post-acute care options, staying informed about community resources.
Qualifications include an Associates or BSN degree, current RN licensure, and preferred healthcare or case management experience.
Requires weekend and holiday availability and proficiency with Microsoft Office.
