Case Manager RN

Job Description

The MSW Case Manager leads complex discharge planning and collaborates with healthcare team members to develop and evaluate safe, proactive discharge plans. They coordinate care, serve as a resource for psychosocially complex cases, and work closely with RN Case Managers on medically complex situations. They are responsible for managing care in accordance with policies, mission, and values, while maintaining knowledge of community resources and post-acute care options.

Qualifications include a graduate degree in social work from an accredited program, with preferred experience in healthcare or case management. Licensure in NC for restrictive interventions within 3 months of hire is required, along with weekend and holiday coverage. Knowledge of Microsoft Office is preferred.
