Job Description
Provides clinical leadership to enhance quality and cost compliance, managing inter-professional teams to deliver comprehensive advanced nursing care aligned with regulations and standards.
Evaluates patient responses, revises care plans, and leads interdisciplinary team rounds.
Performs post-discharge, assessment, palliative, and urgent care visits, including interventions, treatment ordering, education, and discharge planning.
Collaborates with patients, families, and providers to ensure continuous, high-quality care across settings.
Ensures documentation, billing, and compliance with care standards.
Participates in quality improvement, peer reviews, and mentoring.
May provide psychiatric assessments, psychopharmacology, and mental health interventions for psychiatric nurse practitioners.
Maintains licensure, certifications, and professional growth, with preferred experience in home care and familiarity with quality measures.
