Home Care Licensed Practical Nurse (LPN): Experience independence, autonomy, and flexibility while visiting patients in their homes.

Hours: Full-time | Days | 8:00am – 5:00pm

Territory: Fieldwork in the Southwest Twin Cities Metro area, with the office located in Roseville, MN.

Requirements:

  • Current LPN license in MN, active and in good standing.
  • 2 years of LPN experience.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • CPR Certification.

Key Responsibilities:

  • Participate in ongoing patient assessments.
  • Collaborate on care plans with patients and caregivers.
  • Deliver physician-ordered services as per care plans.
  • Report changes in patient conditions to the RN.
  • Educate patients and families on healthcare needs.
  • Document clinical notes accurately and timely.
