Job Description

–PURPOSE AND SCOPE:

Supports FMCNA’s mission and core values, ensuring compliance with regulatory and policy requirements. Provides psychosocial services to in-center and home dialysis patients, utilizing Social Work Theory and practice. Collaborates with the healthcare team to enhance patient rehabilitation and quality of life, while educating staff on the emotional impacts of Chronic Kidney Disease. Actively participates in quality improvement initiatives and adheres to FMCNA’s Compliance Program.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Drives FMS culture through values and customer service standards.
• Maintains effective communication and relationships with all customers.
• Assesses patients’ psychosocial status and collaborates on care planning.
• Provides psycho-educational counseling and community resource referrals.
• Educates patients and families on kidney disease and treatment options.
• Collaborates with financial coordinators to address insurance issues.
• Assists in staff training on psychosocial topics.

EDUCATION:

• Masters in Social Work Required

EXPERIENCE AND REQUIRED SKILLS:

• 0 – 2 years’ related experience.
• State-specific licensure required.
