This role supports the organization’s commitment to quality through chart reviews, peer review processes, and guidance on LHN process improvement and compliance. Collaborating with physicians and healthcare professionals, it ensures care coordination for a high-risk population within the hospital. The position is vital for maintaining safety and high-quality care standards. Responsibilities include coordinating self-management support and disease registry activities, utilizing data to manage population health, and advising on quality incidents. Exceptional interpersonal skills and a positive attitude are essential.

EDUCATION

1. Education: Registered Nurse required; BSN preferred
2. Experience: Minimum three (3) years in a hospital
3. Licensure: Current Washington State RN license
4. Other: Bilingual (English-Spanish) preferred; ability to work with diverse populations

Salary Range: $39.93 – $54.03
