We are seeking a dedicated RN – Case Management to join our team in Eureka, CA.

Responsibilities include:

  • Assessing patient needs and developing individualized care plans
  • Coordinating discharge planning and ensuring seamless care transitions
  • Collaborating with healthcare providers, patients, and families to optimize outcomes
  • Monitoring patient progress and adjusting care plans accordingly
  • Providing education and support to patients and caregivers
