Seeking a Home Health Physical Therapist for an ongoing staff position in Elkhart, Indiana.
Provide high-quality, patient-centered care in the home setting, evaluating needs, developing treatment plans, and adjusting care for optimal outcomes.
Lead and coordinate with support staff, document evaluations and treatments, and promote physical therapy in the community.
Must have a valid PT license, graduate from an approved program, and at least one year of healthcare experience.
Travel within the coverage area (up to 50%) using a dependable vehicle, with physical demands including lifting 50-100 lbs.
