Job Description

Seeking a Home Health Physical Therapist for an ongoing, full-time role in Milwaukee, Wisconsin, with 40 hours/week working day shifts.

  • Responsibilities include conducting patient evaluations, developing and updating care plans, and providing ongoing assessments and interventions.
  • Manage caseloads, coordinate with physicians and team members, and ensure documentation compliance.
  • May supervise PT Assistants and participate in discharge planning and quality improvement activities.
  • Provide patient and caregiver education, assist with adaptive devices, and perform patient positioning and transfers.
  • Must hold a valid Wisconsin or Illinois PT license, BLS certification, and a valid driver’s license.
  • Requires a Doctorate in Physical Therapy, at least 1 year of clinical experience, and proficiency in clinical skills and basic computer software.
  • Physical ability to perform patient transfers, lift up to 50 lbs, and work in various environments is essential.
