Job Description

The Case Manager is an active member of the healthcare team responsible for facilitating the delivery of services through integration of Utilization Management, Case Management, and Discharge Planning roles.

  • They coordinate care and develop discharge plans by collaborating with healthcare professionals and resources.

  • The goal is to promote a collaborative, interdisciplinary approach to achieve optimal clinical and financial outcomes while ensuring compliance with regulations.

    • Qualifications include a minimum of an Associate’s degree (Bachelor’s preferred), and licensure as a Registered Nurse with an active North Carolina or compact RN license.
    Skills required include effective communication, critical thinking, independence, calmness under pressure, and collaboration for discharge planning.
    Optional BLS certification is also noted.
