Job Description

The Case Manager is an active member of the healthcare team responsible for facilitating healthcare service delivery, integrating Utilization Management, Case Management, and Discharge Planning.

The role involves care coordination and developing discharge plans through collaboration with internal and external resources to promote optimal clinical and financial outcomes while ensuring regulatory compliance.

Qualifications include:

  • An Associate’s degree is required; a Bachelor’s degree is preferred.
  • Skills needed are effective communication with patients and families, critical thinking, independence, calmness, and teamwork for discharge planning.
  • Must hold a current/active RN license in North Carolina or a compact state; BLS certification is optional.
