Assess, plan, coordinate, and manage patient care throughout their hospital stay, focusing on discharge readiness and transitions within the acute care setting.
Lead the caregiving team to ensure competent, compassionate care for inpatients, outpatients, and community clients.
Participate in the Nursing Professional Advancement Pathway (NPAP), progressing from novice to competent and potentially to proficient or expert within a specialty.
Maintain confidentiality and security of sensitive information, adhering to HIPAA and relevant regulations, with access based on a âneed to knowâ basis.
