Job Description
This RN position in the Thoracic Surgery outpatient department supports patient care, triage, education, advocacy, and care coordination, including self-management support and transition across settings.
Works collaboratively with the healthcare team to develop and update individualized care plans.
Performs patient assessments, provides age-appropriate education, and facilitates communication among providers and patients.
Coordinates referrals, home care, and follow-up, ensuring compliance with regulatory standards.
Supports regulatory compliance, documentation, and quality improvement initiatives.
Requires RN licensure, CPR certification, strong communication, customer service skills, and experience in ambulatory care preferred.
Must be adaptable, able to operate clinical and office equipment, and handle physical demands of a clinical environment.
