Job Description
This RN role in the Thoracic Surgery outpatient clinic involves providing patient-centered care, education, advocacy, and care coordination for surgical patients.
Supports self-management by assessing needs and enhancing health literacy.
Coordinates care across settings, develops individualized plans, and communicates updates.
Performs triage, educates patients and families, and facilitates referrals and follow-ups.
Assesses patient understanding, manages documentation, and ensures regulatory compliance.
Collaborates with healthcare team, participates in quality improvement, and supports team orientation.
Requires RN licensure, relevant experience, excellent communication, and computer skills. Physical ability to perform clinical tasks is necessary.
