Job Description
Seeking an RN Home Health Case Management position in West Columbia, SC, for ongoing day shifts.
Provide home-based care to patients post-hospitalization or with chronic conditions.
Collaborate with physicians and a multidisciplinary team to assess, document, and report patient progress.
Educate patients and families on care plans, medication, and safety.
Ensure goals are met and coordinate patient discharge.
Must have an active SC RN license, at least 1 year of nursing experience, preferably in home health or critical care.
