Job Description
This position focuses on providing memory support aligned with the Memory Support Program Philosophy, ensuring personalized care for residents.
Collaborates with clinical teams for resident moves and updates resident profiles within a set timeframe.
Conducts orientations and supports educational groups for residents and families.
Coordinates care level changes and prepares related documentation in partnership with wellness and assisted living teams.
Maintains readiness for licensing and accreditation surveys and completes required state training.
Requires 1-2 years of dementia care experience and at least 1 year of supervisory experience.
