Job Description

We’re seeking a Primary Care Physician dedicated to transforming senior healthcare through innovative, patient-centered care.

Responsibilities include providing comprehensive care for patients with acute and chronic conditions, leading care teams, and managing outcomes for panels of up to 450 patients.

The role emphasizes accountability, health coaching, simplifying care actions, and fostering a positive, collaborative culture.

Candidates must have an MD or DO, current medical license, board eligibility or certification in relevant specialties, DEA and BLS certifications, and strong communication skills.

The position requires independent judgment, travel as needed, and a passion for delivering high-quality, evidence-based care that considers lifestyle factors.
