As an MDS Coordinator/Nurse Assessment Coordinator, you’ll complete accurate MDS assessments, contribute to personalized care plans, and ensure proper clinical reimbursement.

Key responsibilities include assessing nursing needs, determining PDPM, monitoring CMI scores, ensuring compliance, collaborating with teams, and providing education.

Qualifications include a valid RN license, experience in long-term care or related roles, knowledge of regulations, and strong organizational and communication skills.
