Conduct comprehensive assessments including medical history, diagnosis, treatment, and social factors.
Collaborate with physicians, case managers, and caregivers to improve patient outcomes.
Work in various settings like home visits and skilled facilities, with flexible hours including days, evenings, nights, and weekends.
Earn from $1,900 to $9,500/month based on assessment volume, with mileage reimbursement.
Qualifications include a Master’s degree or equivalent, current RN and NP licensure, board certification, and CPR/BLS.
Travel required to meet patients at their residences, with a valid driver’s license and insurance.
