Nurse Practitioner – Full Time State Traveler

We are seeking a full-time Nurse Practitioner for travel-based home risk assessments across Ohio and neighboring states.

Conduct adult/geriatric assessments, review medical histories, medication adherence, and social factors.

Collaborate with physicians, social workers, and caregivers to provide comprehensive care and follow-up.

Perform assessments in home, skilled facilities, and community settings.

Work Monday–Friday, 8 am–5:30 pm, with flexible hours and up to 80-90% travel.

Salary ranges from $113,700 to $133,500 annually, plus benefits including health insurance, 401K, PTO, and mileage reimbursement.

Requires Master’s degree or equivalent experience, current RN and NP licensure, board certification, and 3+ years experience.

Must have valid driver’s license, willingness to travel overnight, and ability to work in various environments.
