Seeking a Nurse Practitioner for PRN in-home risk assessments in Warren, OH, with flexible scheduling including days, evenings, nights, and weekends. New grads encouraged to apply, with a $1,500 sign-on bonus.
Conduct comprehensive assessments of adult/geriatric patients, reviewing health, medical history, medication adherence, and social factors.
Collaborate with physicians, case managers, family members, and caregivers to improve patient care.
Work in various settings such as home visits and skilled facilities.
Earn $1,900 to $9,500/month based on completed assessments (pay per assessment).
Qualifications include a Master’s degree or equivalent, current RN and NP licensure, certification in a specialty, and CPR/BLS.
Travel required; valid driver’s license and insurance needed. Benefits include mileage reimbursement and 401(k); benefits for non-benefited providers are limited.
