Seeking a Nurse Practitioner for PRN home risk assessments, welcoming new grads.
Earn from $1,900 to $9,500/month based on visits and assessments.
Conduct comprehensive assessments in patients’ homes, skilled facilities, or community settings.
Collaborate with physicians, case managers, and family members to review health, med history, social factors, and risks.
Provide care in flexible hours including days, evenings, nights, and weekends.
Qualifications include Master’s degree or equivalent, current RN/NP licensure, certification, and at least 1 year experience.
Travel required to patient locations; valid driver’s license and auto insurance needed.
