Seeking Nurse Practitioners or Physician Assistants for PRN home risk assessments in New York.

  • New grads welcome; $1,500 sign-on bonus offered.

  • Conduct comprehensive assessments up to one hour, reviewing health, history, medications, and social factors to improve care quality.

  • Collaborate with healthcare teams, provide patient education, and document findings per protocols.

  • Assessments can be done in home, skilled facilities, or community settings, with flexible scheduling including days, evenings, nights, and weekends.

  • Earn from $2,300 up to $11,500/month based on completed visits; pay is $115 per assessment.

  • Qualifications include a Master’s degree or equivalent experience, active licensure, board certification, current CPR/BLS, and ability to travel.
