Job Description

Seeking a full-time RN Case Manager for hospice care in Washington, NC.

  • Provide direct patient care, coordinate with families and healthcare teams, and manage patient plans of care.
  • Report to the designated supervisor and maintain clinical records.
  • Must be compassionate, flexible, and have RN licensure in NC, with 1+ years of hospice, home health, or related experience preferred.
  • Require a valid driver’s license, CPR certification, and the ability to support patients wherever they call “home.”
